Every year in the U.S., according to a new Environmental Working Group (EWG) study, almost 10,000 deaths in the U.S. are attributed to asbestos exposure. In 1989, the EPA banned the use of asbestos in all new uses and created standards, but any already containing asbestos products remained. According to a 2004 U.S. Geological Survey, asbestos use is 80 percent contained in roofing products, eight percent contained in gaskets, four percent contained in friction products, and eight percent contained in other consumer products. Malignant mesothelioma of the tunica vaginalis

Spiess PE et al., To review our experience with the management of malignant mesothelioma of the tunica vaginalis with emphasis on disease-related outcomes. METHODS: A retrospective chart review of patients seen during the past 25 years at our cancer center identified 5 cases of malignant mesothelioma of the tunica vaginalis. RESULTS: The mean age of patients at presentation was 61.2 years (range 57 to 83). Asbestos exposure was identified in 4 patients. Three patients presented with clinical symptoms suggestive of a hydrocele and two presented with clinical signs of an inguinal hernia. The final diagnosis was established intraoperatively in 1 patient and postoperatively in the remaining 4. Radical orchiectomy or hernia sac with spermatic cord excision was the primary treatment modality. Although radical surgical treatment achieved negative resection margins in 4 cases, 4 of 5 patients died of the disease, with a mean disease-specific survival of only 29 months (range 5 to 68). Regional inguinal lymph node metastasis developed in 3 of 5 patients. Salvage therapy did not prove curative in the 2 patients who received it. CONCLUSIONS: Malignant mesothelioma of the tunica vaginalis constitutes a rare but often fatal malignancy of the male genitalia. This diagnosis should be suspected in patients exposed to asbestos and presenting with clinical symptoms of either hydrocele or inguinal hernia. Frequent inguinal lymph node involvement indicates a potential role of inguinal lymphadenectomy in the primary treatment.

The proportion of peritoneal mesotheliomas among all mesotheliomas has been decreasing, leading some to suggest that peritoneal mesothelioma occurs only after high levels of exposure to asbestos. To investigate the relationship between asbestos exposure and the development of peritoneal mesothelioma, a case-control study examined 40 cases of primary peritoneal mesothelioma from a single institution. This series differed from previous reports in that 75% of the cases and controls had attended college. Malignant mesothelioma of the pleura is a relatively rare neoplasm that has been estimated to account for 20 deaths per million males per year in North America and Europe. A causative association has been well established with asbestos exposure. Paradoxically, the incidence of this tumor continues to rise despite public efforts to reduce, contain or eliminate exposure to asbestos fibers over the past few decades. Another paradoxical feature of the disease is that the majority of malignant mesotheliomas represent morphologically low-grade, well-differentiated neoplasms, yet they follow a relentlessly aggressive and virtually uniformly fatal outcome. For this reason, identification of clinical, morphologic, immunohistochemical or molecular genetic parameters is of extremely limited value for prognostication. Surprisingly, for a disease that currently has no known cure, one of the major problems still lies in establishing the correct diagnosis. Diagnosis acquires a particular relevance in light of the medicolegal ramifications of this disease, and diagnosis of malignant mesothelioma is still fraught with difficulties. Despite the advances in modern diagnostic techniques, no specific markers or morphologic features exist that are exclusive to these tumors. Herein, the current status of malignant mesothelioma diagnosis is reviewed, including the possible contributions of modern molecular techniques for their diagnosis.

A synthetic Bcl-XL antagonist induces apoptosis and chemosensitization in malignant pleural mesothelioma

IntroductionMalignant pleural mesothelioma (MPM) is often amenable to local control via surgical resection and adjuvant radiation therapy. Treatment failure ultimately results from chemotherapy-resistant micrometastatic disease. MPM express the anti-apoptotic protein Bcl-XL, and heterodimerization between Bcl-XL and pro-apoptotic Bcl-2 family members results in resistance to apoptosis and chemotherapy. BH3I-1 is a synthetic Bcl-XL antagonist which inhibits Bcl-XL heterodimerization with pro-apoptotic proteins. We propose that BH3I-1 may initiate apoptosis and chemosensitization of MPM. MethodsHuman MPM cell lines I45 and REN were exposed to BH3I-1 alone and in combination with cisplatin. Cell viability was determined by XTT assay. Apoptosis was assessed by fluorescence-activated cell sorter (FACS) determination of subdiploid populations, and western blot determination of cleavage of caspases 3 and 9. Isobologram analysis was performed to determine if combination therapy was synergistic. ResultsBH3I-1 alone resulted in decreased viability in both cell lines by XTT, and an increase in the SubG0 population compared to controls as determined by FACS [I45: 14.5% vs. 5.2%, REN: 29.2% vs. 5.4%, p<0.05]. Western blot demonstrated cleavage of caspases 3 and 9. Moreover, a subtherapeutic concentration of BH3I-1 sensitized both cell lines to cisplatin-induced apoptosis. Isobologram analysis demonstrated a synergistic effect. ConclusionsExposure of MPM cells to a synthetic Bcl-XL antagonist leads to apoptotic cell death and sensitization to the chemotherapeutic agent cisplatin. Such agents may circumvent resistance to conventional chemotherapy currently experienced in the adjuvant treatment of mesothelioma.

Natural Asbestos (UltraMafic Rock) Found to Cause Mesothelioma

It has long been known that exposure to manufactured airborne asbestos fibers can cause Mesothelioma. A recent report indicates however, that regular exposure to ultramafic rock, the natural source of asbestos, can also have devastating health consequences. While the danger of contracting asbestos through ultramafic rock are low relative to the chances of contracting the disease through workplace exposure, particularly in shipyards, the threat is nonetheless quite real. The disease currently kills some 2,500 individuals per year, a similar number as those reportedly killed by second hand smoke. Researchers from the California Cancer Registry argue that although the danger of exposure through natural causes is more remote than the possibility of contraction through a workplace environment it was still an important risk. In fact, the research showed that the liklihood of contracting mesothelioma increased remarkably with ones proximity to deposits of ultramafic rocks. Such rock deposits can be found throughout the Sierra Nevada and throughout northern and central California. Specifically, the odds of having mesothelioma fell by 6.3 percent for every 10 kilometers (about 6.2 miles) farther a person lived from the nearest asbestos source. The association was strongest in men, but was also seen in women.

He also reported from around the world. Mister Jennings was sixty-seven years old. Since his death, many people have questions about lung cancer and how to prevent it. Most lung cancer deaths are caused by tobacco use. Peter Jennings smoked cigarettes for many years. But smokers are not the only ones at risk. So are people who breathe tobacco smoke in the air. Radon gas in the environment, particles of the fire-resistant material asbestos and air pollution also increase the risk. More people die of lung cancer than any other form of cancer. Each year, more than one million die of the disease. Once it is found, more than ninety percent of patients are dead within two years. People often do not show signs until the cancer has spread to the brain, liver or bones. Then it is usually too late to cure. Signs of lung cancer include a cough that gets worse and pain in the chest area. People may cough up blood and lose their normal voice. Weight loss and feeling tired are two other signs. The American Cancer Society says lung cancer is most often found when people reach their seventies. It is generally rare in people under the age of forty. There are two major kinds: small-cell and non-small-cell lung cancer. Non-small cell lung cancers are the most common. These usually spread at a slower rate to other parts of the body. Doctors can usually remove a lung cancer if it is found early. Other treatments involve radiation or chemotherapy drugs, or both. Lung cancer is most common among smokers. Non-smokers, however, are more likely to have a kind of lung cancer that is linked to genetics. Experts say new drugs offer better treatment for this form of lung cancer. Two days after Peter Jennings died, Dana Reeve announced that she has lung cancer. Her husband was Christopher Reeve, the actor who was thrown from a horse and broke his neck. Too Much Prostate Cancer Treatment

Aug. 15, 2006 -- More than half of men with low-risk prostate cancerprostate cancer are overtreated with surgery or radiation therapy, a University of Michigan study shows.David C. Miller, MD, MPH, and colleagues looked at National Cancer Institute data on 24,405 men with lower-risk prostate cancer. These are men of any age with low-grade tumors or men aged 70 or older with moderate-grade tumors.Low-grade tumors don't always become dangerous. And if they do, there's still time to act before they turn deadly. Men over 70 with moderate-grade prostate tumors are most likely to die of something other than prostate cancer over the 20 years after diagnosis.Miller's research team found that 55% of men with these low-risk prostate cancers underwent surgery or radiation treatment within a year of diagnosis.Miller and colleagues note that early treatment for prostate cancer helps many men. But for many other men, early surgery or radiation therapy means suffering bothersome side effects yet getting little benefit in return."It is clear that the number of lower-risk patients who receive initial aggressive therapy is not trivial," Miller said, in a news release. "We have to ask the question whether this is too much treatment for some of these men."Miller and colleagues note that men who learn they have cancer -- and their doctors -- often want to take an action-oriented response. But acting too fast, they suggest, is as bad as acting too slowly."In our view, if the treatment decision is inappropriate for an individual patient, then no matter how skillfully surgery is performed or radiation is delivered, it is poor-quality treatment," they conclude. A spokesman said doctors expected a full recovery. The prostate is part of the male reproduction system. It is a small gland below the bladder. Cancer of the prostate usually develops after the age of fifty. The disease can kill if it spreads. But it can be cured if discovered early.Doctors can do a blood test to find prostate cancer. The test measures the amount of a protein called prostate-specific antigen, or PSA. An injured or diseased prostate lets PSA into the blood. The more PSA that is found, the greater the chance that a man has prostate cancer. If a man has a high PSA level, a doctor will remove small pieces of the prostate and examine them. Men with early prostate cancer have some choices. One is an operation to remove the gland. Another is radiation treatment. Experts say radiation is a good choice for men who are too old or weak for the operation.Both treatments are considered the most effective interventions for early prostate cancer. However, they can affect a man in other ways. They can make him unable to perform sexually. Another possible effect is a loss of control over the release of urine from the bladder. Doctors say some men with early prostate cancer do not need any immediate treatment. They suggest this for old men or those with other serious health problems. Prostate cancer generally grows slowly. Treatment can be started later if the cancer is found to be growing. Prostate cancer that has spread can be treated with anti-cancer drugs or hormones. Prostate cancers grow in the presence of testosterone.
